My thoughts

Big picture, if your priority is covering long-term care costs, Mutual of Omaha provides the highest benefits. But Nationwide provides a number of other perks, namely a large death benefit if you never need care and cash payouts. See our reviews on each for more insights.

I chose these policies based on your preferences:

Memory care: No
Alzheimer’s care can last 8+ years—longer than average—but it's less common, and longer policies cost more. That’s why I chose shorter, more cost-effective options for you.

Refund: No
I chose two policies that don’t offer a refund—these are often called 'pure LTC insurance' because they only pay for care and are usually more affordable. I also included one with a refund option (Nationwide) for comparison. With Nationwide, if you don’t need care, your heirs get a life insurance payout. It’s a nice perk—unlike auto or home insurance, you can actually get money back if you don’t use it.

Smaller payments over time: Unsure
Smaller annual payments are easier on the budget but cost more in the long run. I chose three policies with mid-range payment schedules. Shortening the payment period will increase your benefits.

You asked about seeing multiple payment options. Mutual of Omaha only offers lifetime payments. For Nationwide, if you paid over 5 years instead of 10 years, you'd see a roughly 15% increase in your annual/total benefits. I'd be happy to provide updated quotes on any policy.

You'll notice the benefits line is flat. This policy grows at 3%, but once you're on claim (receiving benefits), the benefit pool stops growing.

Comparing to LTC costs
The graph shows your benefits are below projected LTC costs—but you don’t have to cover everything. In assisted living, housing and meals are often included, and other income or assets can help. A 10% increase in premiums will increase your benefits by 10%.
